Dental Premier, Delta Dental PPO, Denta Select, Superior Dental Care, Preferred Health, Dental Wellness Partners, Health Resources and any dental insurance that is a part of Connection Dental. If your insurance is not listed
above, please call us to see if we are in network.
As a courtesy to you, we will work with your insurance company provided it processes predeterminations, assigns benefits to our office and that there is not an out of network issue.
We may predetermine insurance benefits prior to treatment so that you will know your cost share. you will be responsible for any amounts that your insurance company does not pay or according to the network contract.
In order to process your dental benefits, we must have your insurance information prior to your appointment. Please provide the required information in the insurance section of the new patient form before returning it to Dr. Frederick Swain, 4010 Dupont Circle, Suite 529, Louisville, KY 40207. We accept checks, credit cards and cash on the day of service and select insurance plans. We offer payment options to work within your budget on a preplanned basis. Please see our financial coordinator for details.
